Alpha-Lipoic Acid for Nerve Pain: Dosage and Side Effects

Alpha-lipoic acid has a clinical record most supplement ingredients do not: a named series of randomised trials, ALADIN, SYDNEY, ORPIL and NATHAN, testing it in diabetic peripheral neuropathy. It is a prescription treatment for that indication in Germany. Its use in general antioxidant and weight products is a long way from the evidence that gives it credibility.

How Alpha-Lipoic Acid Works

A mitochondrial cofactor for pyruvate dehydrogenase and both a water- and fat-soluble antioxidant, which lets it act in membranes and cytosol alike. It regenerates vitamins C and E and glutathione, and improves endothelial nitric oxide availability. In neuropathy the working hypothesis is reduced oxidative damage to peripheral nerves.

  • Diabetic peripheral neuropathy: 600 mg/day orally for up to five weeks improved symptoms and signs without significant side effects, with meta-analysis support across the ALADIN and SYDNEY trial families.
  • Nerve conduction velocity: improved at 300-600 mg/day intravenously in a meta-analysis of ten trials covering 1,242 patients.
  • Insulin sensitivity: modest improvements in some trials.
  • Body weight: small reductions in meta-analysis, not the reason it is worth taking.

Research Summary

Ziegler 2004 meta-analysis of alpha-lipoic acid in symptomatic diabetic polyneuropathy, a 2011 critical appraisal pooling 1,160 participants across ALADIN, SYDNEY, ORPIL, SYDNEY 2 and ALADIN III, and a 2023 meta-analysis and systematic review of oral treatment. The oral 600 mg/day dose is where the practical evidence sits.

600 mg/day orally is the dose the neuropathy literature converges on, taken on an empty stomach because food reduces absorption by around 30%. Doses above 600 mg have not shown additional benefit in the trials that looked. The R-enantiomer is the biologically active one; most products sell a racemic mix of R and S.

Side Effects

Nausea, rash and a metallic taste. It can lower blood glucose, which matters for anyone on glucose-lowering medication. Rare reports of insulin autoimmune syndrome, mostly in people of Japanese and Korean descent carrying particular HLA types.

Interactions

Additive with insulin and oral glucose-lowering drugs, requiring glucose monitoring. Chelates metals, so separate it from iron, magnesium and calcium supplements by several hours. May interact with thyroid medication. Theoretically reduces the efficacy of chemotherapy that works through oxidative mechanisms.
